Mechanism:

Sufentanil is a synthetic, potent opioid with highly selective binding to mu-opioid receptors. Sufentanil acts as a full agonist on these receptors and does not induce histamine release. It is 5-10 times more potent than fentanyl and is the most potent opioid. However, it has a shorter duration of action. Sufentanil has a high lipid solubility, which accounts for the fast onset when given intravenously. It is also a weak inhibitor of serotonin reuptake transporter (SERT) by a direct, non-opioid receptor mechanism.

Application:

Sufentanil is an opioid analgesic that is used as an adjunct in anesthesia, in balanced anesthesia, and as a primary anesthetic agent. Although it is a very strong analgesic, being as much as 10 times as potent as fentanyl, its main use is in operating suites and critical care where pain relief is required for a short period of time. Nevertheless, it has been marketed as a S/L analgesic in the UK and is under clinical trials as a transdermal patch.
